The CVS Caremark Charitable Trust and the National Association of Community Health Centers have announced the launch of a $3 million, multiyear initiative designed to help Americans manage chronic diseases.
Through the Innovations in Community Health initiative, grants will be awarded to community health centers across the country working to develop innovative programs that increase access to quality care and produce better health outcomes while reducing costs for patients and healthcare systems. The funding also will support healthcare models that promote coordinated care and/or include education and tracking models for promoting medication adherence among patients managing chronic diseases, and technology and IT services that enable providers to plan therapies more effectively and promote communication among a network of other centers serving chronic disease suffering patients.
