Temporary Case Manager

Main Areas of Responsibility: 

Temporary Case Manager for the Lifting Mothers out of Poverty Program- (Alb, NM/Temporary FT for 1 year and contingent on continued funding). We are seeking a compassionate and experienced Case Manager to provide case management, resource coordination, and goal-setting support for low-income pregnant women and mothers with infants.

Additional Areas of Responsibility: 

Bachelor's degree in social, behavioral or health science or a satisfactory equivalent combination of education and experience; 1 year experience in case management or social services. Strong communication, problem-solving, and organizational skills, along with experience in social services, client advocacy, and resource navigation. Proficiency in MS Office and the ability to collaborate with service providers and community partners are essential.

Other Information/Requirements: 

Strong understanding of resource coordination, workforce development, and client advocacy.  Knowledge of trauma-informed care and evidence-based case management approaches.  Ability to collaborate with service providers, government agencies, and community partners. A valid NM driver's license with a clean record.  Knowledge of the local social services system and bilingual in English/Spanish is preferred.
