Main Areas of Responsibility:
Full time position. Assist families with infants/toddlers who have developmental delays/disabilities to access and coordinate early intervention services for their children in accordance within state/federal requirements. Collaborate with early intervention team, health care providers, school and other community resources. Provide relationship based, family education services that support the child and family during their daily routines in their home/community.
Additional Areas of Responsibility:
Conduct and integrate psychosocial assessment, behavior observation and other assessment data to reflect a holistic view of the child and family. Conduct counseling, crisis intervention and related social work services. Must be able to drive to provide in-home and community based services to Bernalillo, Rio Rancho, Paradise Hills & Ventana Ranch areas.
