Main Areas of Responsibility:
Provide comprehensive case management services from screening and intake, including client assessments, development of self-sufficiency plans, coordination of services, and discharge and after-care planning.
Collaborate with clients to identify personal goals related to education, employment, housing, mental health, and substance abuse recovery, and develop action plans to achieve these goals.
