Intensive Case Manager

Main Areas of Responsibility: 

Casa de Salud is hiring for a full-time Intensive Case Manager who is passionate about assisting individuals with co-occurring substance use and mental health needs. The ICM will provide education, care coordination, and access to community resources.

Additional Areas of Responsibility: 

Coordinate and monitor services. Assist clients in developing and meeting goals. Assess patients and participate in treatment planning with patient care providers. Document, Maintain and report on client services. Additional job-related duties as assigned

Other Information/Requirements: 

The ICM would work on site at Casa de Salud as well as in community settings, including with clients at their homes. At least 2 years prior experience working with populations affected by addictions, behavioral health issues, homelessness, discrimination, persons that have experienced violence either as a victim or a perpretrator.
