Job Description
Main Areas of Responsibility: 

Intake Coordinator for our Behavioral Health Department. Responsible for initial assessments to determine eligibility for SMHC Behavioral Health Programs and for providing case management and referrals until disposition is determined. This work is done on-site and by outreach to other providers.

Other Information/Requirements: 

Prefer Master's level with ability to diagnose.
Experience with SMI and homeless population required.
