Main Areas of Responsibility: 

Initiate and maintain a therapeutic alliance with CRT Clients to assist clients achieve treatment and life goals leading ultimately to client independence from HopeWorks CRT services. ^0 % of client appoints are in person with monthly home visits with clients housed through a supportive housing voucher. Complete strength based quarterly Treatment plans and interim reviews on a quarterly basis. Complete crisis and discharge plans as well as HIPAA paperwork annually.

Additional Areas of Responsibility: 

Administer drug/alcohol tests as needed and link Clients to necessary psychiatric, substance abuse, medical, dental and/or community services which is mutually agreed upon by Client and CSW. Assist with entry into therapeutic group, psychosocial rehabilitation, vocational, volunteer or educational programs. Bill 300 units per month complying with all Medicaid/Falling colors rules regarding allowable billable opportunities. Duties may change based on Agency needs.

Other Information/Requirements: 

Bachelor's Degree, preferable in a human service-related field or High School Diploma with three years experience in a human services field. At least one year of experience in case management or direct care working with SDMI population or other relevant experience.
