Community Support Worker

Full-Time
Main Areas of Responsibility: 

Provide person-centered, comprehensive case management/community support services to individuals with mental health and/or substance use diagnoses.

Additional Areas of Responsibility: 

Monitor and coordinate treatment, linkage to community services, crisis intervention, advocacy, comprehensive care planning and skill-building activities, as well as appropriate and timely documentation.

Other Information/Requirements: 

Requires a minimum of high school diploma or GED and 3 years' experience. Preferred: Bachelor's degree in Human Services field (Social Work, Psychology, Counseling) and 1 year relevant experience, or Master's degree in human-services related field. Other requirements: motor vehicle records check, drug screening, and criminal background check.
