Main Areas of Responsibility:
Independently provide ongoing and direct community support services to individuals with severely disabling mental illness as part of an inter-disciplinary treatment team. Spend considerable time (at least 60%) in the community to engage/coordinate support services (family, physicians, psychiatrists…) for each client as needed.
Additional Areas of Responsibility:
Perform variety of casework duties: analyzing/assessing clients’ psychosocial & multi-systemic needs; level of family function; individual clinical service/treatment plans; implement services; provide advocacy. Develop/maintain effective working relationships with other agencies/institutions. Initiate & complete documentation: mini-mental status exams, daily progress notes, transfer summaries, & provide input in development of clinical service/treatment plans. Complete client billing on weekly basis.
