Main Areas of Responsibility: 

We are seeking two dedicated Housing Case Managers with a strong passion for serving homeless communities. The ideal candidate will work closely with individuals and families experiencing homelessness, providing crucial support in navigating applications for assistance to achieve and maintain stable housing.

Additional Areas of Responsibility: 

Engage regularly and consistently with housing participants to deliver comprehensive wraparound services.  Apply principles of Housing First, Harm Reduction, and Trauma-Informed Care in all interactions.  

Other Information/Requirements: 

Maintain accurate records and case notes using HMIS (Homeless Management Information System). Utilize MS Office applications, including SharePoint, to manage and track participant progress.
