Main Areas of Responsibility:
The South Valley Economic Development Center is looking for a full-time community outreach coordinator to seek out individuals in the community who are unconnected to health and wellness services (such as health insurance, dental, child care, transportation, food security, immigration/legal services, alcohol abuse programs, and job placement services), and help them obtain those services while identifying and addressing barriers in the health care system.

Duties:
- Identify health and welfare needs for clients
- Network and map direct services available to clients, and create partnerships to facilitate those services
- Guide clients to partner organizations that can help with those needs
- Follow-up with clients, maintain relationships, and help them reach desired health outcomes
- Document client information and progress using a web-based system
