Mission/Giving Statement: Small grants are given to promote small businesses and education with an emphasis on crafts and arts primarily within the state of New Mexico.
Application Instructions:
Date
Organization/Individual's name, address, telephone, fax, email, and website address, and name of person who is head of organization.
Federal ID number and IRS Determination Letter (as evidence of tax exempt status) for the organization or Social Security Number.
If no tax-exempt status, state name, address, telephone, fax, email and website addresses, and Federal ID number of your fiscal agent.
Project Manager's name, title, and address (if different from above).
State the purpose of your request (example: operating funds, project, program, etc).
Grant amount requested.
Total project cost and budget if different than organization's budget.
Start and end date of the project or grant period request.
Annual organization or projected budget.
State who will be served.
State geographic area that will be served.
Provide a 50 word or less summary of funding request.
Provide a 50 word or less summary of your evaluation process, and what results you expect.
Also provide organization Information: Describe the history / mission / purpose of your organization including its major accomplishments; Provide a list of your board of directors or trustees, and a description of their relevant background and experience; Provide a list of all other sources of organization support; Enclose recent press coverage if any; Enclose three copies of the organization’s most recent audit OR your most recent reviewed or compiled financial statements AND three copies of your most recent Form 990, or that of your fiscal agent.
A full description of the proposed project or purpose requiring operating funds is required:
Provide a description of the project, including goals and objectives.
What determined the need?
Was your constituency involved in the planning process? How?
Will it help woman become more financially self-sufficient? If so, how?
What are the anticipated outcomes?
How will you evaluate and measure its effectiveness?
What are your plans for continuation past stated grant date, including funding?
Include or enclose information about principal staff and/ or volunteers who will implement the program or project.
