Senior Case Manager/SQMRP

Job Description
Main Areas of Responsibility: 

Supervision includes oversight of all case management activities as well as personnel management. The SQMRP carries a caseload of not more than 25 participants, is responsible for counseling & providing linkage to appropriate services for these individuals and their families/guardians. The SQMRP leads the staff in developing Individualized Service Plans (ISP) and leads the process for ensuring that the team comes to a resolution on the appropriate person centered vision.
