Continuing Care Case Worker

Job Description
Main Areas of Responsibility: 

Established non-profit hiring for PT (20 hrs/week through June 2011,) Continuing Care Case Worker. Under indirect supervision the CC Case worker provides case management services, crisis intervention and emergency services, facilitates groups and provides discharge transition services. The CC Case worker administers client surveys and compiles outcome data while maintaining documentation as required.

Other Information/Requirements: 

Requirements: Bachelor’s Degree in social work or a combination of relevant education, training, and experience totaling 4 years. Has an understanding of the dynamics of substance abuse, mental health issues, homelessness, interpersonal violence. Willingness to work with other agencies and develop collaborations for service provision. Access to own vehicle and valid New Mexico Drivers license. Twenty-one years of age. Must obtain background clearance. Bilingual (English/Spanish) a plus.
