Case Manager

Job Description
Type of Job: 
Full-Time
Main Areas of Responsibility: 

The Case Manager will provide direct case management services to individuals participating in Heading Home programs. The Case Manager will be the frontline staff person to respond to client needs through the various supportive services and behavioral health referrals and interventions. The Case Manager will provide aid and comfort to a person in crisis.

Additional Areas of Responsibility: 

Provides intensive case management and services coordination, focused on housing stability.
Assists clients with securing appropriate resources for mental health services and treatment and substance use management
Assists clients with money management and other independent living skills training and assistance
Connects clients to education and vocational training as well as career/job counseling and development
Maintains accurate records of services provided, completed referrals, and clients' files

Other Information/Requirements: 

Trained to provide Trauma-informed Care
Trained in Motivational Interviewing, De-escalation, Conflict Resolution, and Crisis Intervention
High school or equivalent is required
A degree in Social Work, Education, Counseling, Psychology, Sociology, Criminal Justice, Family Studies or a related degree is preferred
One year of any combination of experience is required, three years of direct experience is preferred

Salary Range: 
$17 - $22 Hourly
