Patient Services Coordinator

Job Description
Main Areas of Responsibility: 

This position is responsible for responding to all patient and family inquiries for information about ALS. Maintaining a case load of 40-50 patients and families living with ALS. This requires monthly calls and when needed home visits. Maintain updated information on ALS research and legislative issues to communicate to client base. Contribute to and make recommendations to the Patient Services Committee and ALS Association Newsletter.

Additional Areas of Responsibility: 

Provides in-service education programs to local home health agencies, hospitals, and skilled care facilities
Represents the Chapter at an affiliated Center or ALS clinic, if required
Participates in continuing education seminars, workshops, and conferences, as available and necessary
Other responsibilities as assigned

Other Information/Requirements: 

Preferably a Registered Nurse (RN), Social Worker (MSW) or medical health or social services professional. Health Educators, Occupational Therapists, Physical Therapists or Speech Therapists may be considered
Ability to assess both physical and psychosocial needs of ALS patients and families, based on knowledge of ALS, the disease and its ramifications
The right candidate will also possess knowledge of community resources, strong organizational skills, and strong communication skills.
