Community Support Worker

Job Description
Main Areas of Responsibility: 

Under direct supervision provides assistance to the clinical and administrative staff including program support and clerical assistance. Provides community support services for graduate level clients, crisis intervention and emergency services. Assists with ongoing psychosocial assessment, participates in development of treatment plans including discharge planning, and supports clients in fulfilling treatment plan and achieving goals.

Additional Areas of Responsibility: 

The Community Support Worker helps facilitate the continuation of client and community support services and necessary administrative duties during non-traditional working hours. They are to foster maximum self-determination on the part of the client, to provide crisis intervention, advocacy, and individual life skills education. Help clients with positive goal setting. Answer the after-hours crisis line.

Other Information/Requirements: 

Bachelor's Degree in social work or a combination of relevant education, training, and experience totaling 4 years. Has an understanding of the dynamics of substance abuse, mental health issues, homelessness, interpersonal violence and harm reduction. Excellent written, verbal and interpersonal communication skills. Strong attention to detail, well organized and a commitment to treating others with respect. Experience with Microsoft Office applications (Word, Excel, and Access).
