Main Areas of Responsibility:
Provide case management services to people experiencing homelessness. Responsibilities include providing resources and referrals, completing assessments, developing treatment plans, conducting home visits, writing case notes and transporting clients.
Additional Areas of Responsibility:
Requires a BA in a social services field or equivalent experience and two years experience working with underserved populations. Successful candidate should have a working knowledge of EHR and experience with CMI and DSM-IV. Must have a valid NM driver's license and clean driving record. Bilingual (English/Spanish) strongly preferred.
