Main Areas of Responsibility:
Conduct intakes, assessments, need determinations for clients * Assess barriers to care; engage clients in creating solutions to financial, practical, social challenges * Develop caregiving and respite care plans with clients * Connect clients to public and private services * Empower clients to improve health and well-being using self-care tools, coping strategies, stress management skills * Create relationships with service providers and patient/care navigators that engage family caregivers.
Additional Areas of Responsibility:
Maintain working knowledge of current services available * Conduct community outreach; build relationships with related organizations, disease-related orgs, disability-related orgs, community navigators, community-based orgs, and governmental orgs * Maintain updated resource database with support from volunteers and professional team * Monitor and document client progress to assure that services are received and effective * Conduct reporting for the purposes of grants and organizational evaluation.
