Main Areas of Responsibility:
Provides direct case management services as needed, to include screening referrals for clients in need of one-time assistance.
Collaborates with the Program Manager to ensure that services are delivered in accordance with contractual agreements and regulatory standards.
Ensures that case management services operate efficiently with respect to the staffing ratios and coverage for planned and unplanned time off.
Additional Areas of Responsibility:
Identifies performance issues and assists staff in performing to their highest standard.
Monitors the timely completion of required assessments, Individual Service Plans, and case notes.
Acts as a mentor and resource to staff when issues or crises arise.
Coordinates staff training as needed.
Works collaboratively with Executive Leadership and other housing and administrative staff in planning, developing, implementing and evaluating programs to support and enhance client care.
